Service Coordination helps people and their families find, utilize and coordinate resources and opportunities in the community on the basis of individual needs.
Skilled Service Coordinators independently, advocate and assist people in achieving a balanced life. A life rich with choice, family, wellness, spiritual, culture, community service, school, work, retirement, hobbies and friendships. The underlying values are inclusion, community involvement, and self-determination (allowing people with disabilities and their families to make decisions for themselves, based on informed choice).
